Fallon Health’s Summit ElderCare® is a Program of All-Inclusive Care for the Elderly–PACE for short. • Certification as a Home Health Aide or Nursing Assistant is desired. Reliable Transportation Required.   

• One year of experience working with a frail and/or older adult population, preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently.
• Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis.

• In accordance with the Massachusetts Wage Transparency Act, the pay for this position is $21.50 per hour weekdays  - $24.00 per hour weekends  which reflects what we reasonably and in good faith expect to pay at the time of posting. Final compensation will depend on the candidate’s experience, skills, and fit with the role’s responsibilities.
