Responsibilities
~1 min readIn this role, you will support Southwark’s @Home/Urgent pathway, a hospital admission avoidance service. You will intervene with residents who can be supported home from hospital emergency departments (ED) and those at risk of admission from home. Your responsibilities include assessing patients' needs, coordinating care plans, and ensuring a safe transition back to their homes. You will collaborate with healthcare professionals to provide comprehensive support, aiming to reduce unnecessary hospital admissions and improve patient outcomes.
