Occupational Therapist - Lifelong Services Reviewing

• →Conduct strengths-based reviews and develop support plans.
• →Perform Mental Capacity Act assessments.
• →Contribute to Multi-Disciplinary Continuing Health Care assessments.
• →Collaborate with the reviewing service team.
• →Carry out risk assessments and provide recommendations for assistive equipment and home adaptations.

• Qualified Occupational Therapist with HCPC registration.
• Minimum 2 years of experience.
• Enhanced DBS check.
• Ability to travel as required.

• Strong communication and organizational skills.
• Proficient in case management and report writing.
• Ability to work under pressure and manage multiple referrals.
• Experience with community settings and social care services is desirable.
