Social Worker - Front Door Team

• →Conduct timely and proportionate initial assessments of service users and their carers to identify their needs and desired outcomes.
• →Provide immediate support and intervention to adults in crisis to stabilize their situation and prevent unnecessary hospital admissions.
• →Develop and implement individualized care plans that promote independence and recovery, linking service users to community-based services that enhance personal wellbeing.
• →Work as part of multi-agency teams to provide seamless health and social care services, ensuring effective communication and coordination.
• →Involve service users and carers in creative support planning, encouraging choice and control through the use of personal budgets, including Direct Payments as the default position.
• →Collaborate with Community Development Staff to help service users access community-based services and resources.
• →Adhere to risk management and adult safeguarding procedures in social work practice.
• →Conduct assessments and reviews of service users to a high standard, ensuring they reflect the needs and outcomes of the service user and carer.

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
