Are you an experienced Social Worker looking to make a real difference to young people transitioning into adulthood?

Our client in Bristol city is looking for a skilled Social Worker to join its dedicated Transitions Team, supporting young people aged 14-25 as they move from children's to adult services. Working within a multidisciplinary team of Social Workers, Occupational Therapists and Social Care Practitioners, you'll play a key role in helping young people achieve greater independence and positive long-term outcomes.

Responsibilities

In this role, you will:

  • Complete Care Act assessments, support plans and reviews
  • Support young people and families through the transition to adult services
  • Promote independence through strengths-based practice and early intervention
  • Commission services and identify community-based support, assistive technology and equipment solutions
  • Work collaboratively with health, education and social care professionals

Requirements

To be successful in this role you must have:

  • Social Work England registration
  • Experience completing Care Act assessments and support planning
  • Strong understanding of transitions and strengths-based practice
  • Ability to manage a complex caseload in a fast-paced environment
  • Minimum 2 days per week in the office
  • Wednesday is the designated team day
  • Travel outside Bristol may be required to visit young people placed out of county
