We are seeking a talented Senior Adult System of Care (SOC) Manager to join our team of nearly 500 employees. CCSI is contracted by Onondaga County to provide program management services, including specialized staffing. 

Onondaga County is actively embarking on the Adult Behavioral Health System of Care project. A System of Care (SOC) is a coordinated network of formal and informal services and supports working under shared value set to support systems in being successful in recovery services. The target population is the dual recovery population experiencing challenges related to substance use, mental health, serious emotional disturbances, and developmental disabilities and navigating barriers accessing services. 

The Senior Adult SOC Manager, under the guidance and support of the Onondaga County Mental Health Services’ Division Deputy Commissioner and Director of Community Services, will focus on the high need population, ensuring the system is adequately set up to meet their needs. There is a particular focus on building out the system for individuals experiencing mental health and substance use co-occurring challenges. The Senior Adult SOC Manager will need to establish positive and professional relationships with state agencies, local providers, and organizations while engaging adult serving systems in responding in a collaborative, culturally responsive, trauma informed, cross systems and strengths-based manner.

CCSI is partnering with Onondaga County to hire a Senior Adult System of Care (SOC) Manager. This position is employed by CCSI and working on behalf of Onondaga County.

Responsibilities

  • Act as an educator, convener, and information transmitter, both up to local policy/decision makers and down to front line workers and their supervisors. 
  • Facilitate, coordinate and manage cross systems solution focused meetings for providers working with individuals are caught in systemic challenges or barriers. 
  • Convene multi-system committees or groups focused to track and explore local emerging trends of unmet needs that are indicative of systemic failure or need for system change and ensuring that this information is communicated to Onondaga County Division of Mental Health leadership and other stakeholders as defined.
  • Provide technical assistance to providers and partners on system collaboration on complex cases and propose implementation solutions. 
  • Facilitate cross—discipline trainings to promote the integration of services focusing on Evidenced-Based Practices for individuals with co-occurring disorders. 
  • Work with the Department of Children and Family Services and the larger LGU to monitor providers to ensure the use of integrated, person-centered service and treatment planning. 
  • Input and maintain records in designated system within the established timeframes and maintains hard copy records, statistics, other as directed 
  • Create a protocol to measure outcome improvements in the local service system for individuals with co-occurring disorders.

EDUCATION AND EXPERIENCE:
  • Masters in Human Services field plus 5 years of relevant professional experience and/or some advanced hours of training, OR equivalent education and work experience (see CCSI's equivalency chart)
