✓Reviews clinical documentation to help determine home health appropriateness.
✓Assists in compiling and reporting on clinical metrics shared with community partners.
✓Tracks and updates team members on new hospitalizations, hospital holds and resumptions of care.
✓Must be self-driven with a high degree of attention to detail.
✓Communicates with patients, families and caregivers regarding home health services.
✓Coordinates with physicians and referral sources (as appropriate) regarding home health plans of care.
✓Assists in ensuring that patients are seen in a timely manner and works with physicians to receive updated start of care orders when necessary.
✓Cross trains to provide back-up coverage for scheduling and intake coordination when needed.
✓Assist with field visits if needed.
✓Uses equipment and supplies effectively and efficiently.
✓Participates in personal and professional growth and development.
✓Other tasks and responsibilities as assigned.
✓Graduate of an accredited practical nurse or vocational nursing program.
✓One (1) year nursing experience in home care, hospice care, institutional hospital or facility care preferred.
✓Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of service.
✓Complies with accepted professional standards and practice.
✓Demonstrates good verbal and written communication, and organization skills.
✓Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
✓Possess and maintains current BLS CPR Certification.
