→Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.
→Implement treatment and discharge plans in collaboration with the interdisciplinary team.
→Provide crisis intervention and supportive therapeutic interactions (individual, group, family) under guidance of senior staff.
→Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.
→Identify patient and family needs (emotional, social, financial) and escalate complex or high-risk barriers to senior case management staff.
→Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards

Required Education, Skills and/or Experience:

→Master’s Degree in Social work or a Master’s Degree in a recognized mental health field
→1 year of experience in behavioral health environment preferred
→If unlicensed, must be eligible to sit for licensing exam within the next 6 months as applicable. Must have obtained Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage Family Therapist (LMFT) licensure within one year of start date.
→Preferred - Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT).

→Current CPR/BLS Certification issued by American Heart Association within 90 days of hire
→CPI (Crisis Prevention Intervention) certification within 90 days of hire
