Functions as a part of the hemodialysis health care team as a Charge Registered Nurse to ensure provision of quality patient care on a dialysis basis in accordance with policies, procedures and training. Supports the commitment to the Quality Enhancement program and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhances the likelihood that patients will achieve the Quality Enhancement Goals (QEP). Adheres to all requirements of the Compliances Program, and patient care and administrative policies.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Responsible for driving the culture through values and customer services standards.
  • Accountable for outstanding customer service to all external and internal customers.
  • Develops and maintains effective relationships through effective and timely communication.
  • Takes initiative and action to respond, resolve, and follow up regarding customer services issues with all customers in a timely manner.
  • Demonstrates applicable knowledge of policies and procedures related to the Registered Nurse/LPN/PCT/CCHT for hemodialysis
  • Demonstrates ability to perform technical skills for the Registered Nurse including the patient's treatment, water treatment facility and emergency procedures, as determined by the competency assessment program.
  • Share responsibility of chart auditing with the RN
  • Delivers age specific care to assigned patients as prescribed, including all aspects of the hemodialysis dialysis procedures specifically trained to perform in accordance with state licensure
  • Demonstrates ability to work with any patient or staff, regardless of race, gender disease process, life-style, religious or cultural beliefs, or treatment
  • Obtains, writes, transcribes and notes physician's orders as needed, and follows through appropriately
  • Initiates, monitors, and discontinues dialysis treatments according to established procedures. Assesses patient's pre/post treatment and reports problems to the manager and or Charge Nurse
  • Establishes priorities of care, both emergent and non-emergent
  • Develops and assimilates technical knowledge with observations, assessment, and communication skills to detect dialysis and non-dialysis related problems
  • Recognizes and utilizes opportunities for patient and family education and training
  • Maintains a safe environment for self, patients and staff by utilizing proper body mechanics, and following universal infection control precautions and cleaning procedures under OSHA regulations
  • Dialyzes isolation patients utilizing isolation precautions and prescribed procedures
  • Assess patient's pre/post treatment and reports problems
  • Performs hemodialysis access care according to established procedures
  • Administers and documents medications as authorized by a physician and as further authorized or limited by the State Board of Nursing Practice
  • Reports any significant information or change in patient condition to Unit Manager and Physician.
  • Documents clearly, completely and accurately all pertinent information on patient's manual and/or electronic record
  • Participates in the formation, update and revision of nursing care plans, including long and short term goals, according to established guidelines.
  • Performs or assists in the performance of all emergency operational procedures, including the evacuation of patients in the event of a fire and/or disaster
  • Ensure appropriate preparation of lab requisitions for lab
  • Ensure correct lab tubes are utilized for prescribed lab specimens and that lab draw and processing procedures are performed appropriately for all lab samples
  • Updates medication lists for patients, then prints and puts in chart
  • Monthly checks of meds and crash cart for expired meds
  • Care plans for patients following the listed provided by the Unit Manager
  • Bimonthly medication changes adjusted by lab results, prints new labels and follows all needed items to ensure all changes are done
  • Assists with and performs all procedures of equipment maintenance, safe operation, disinfection and inventory control according to departmental policy, manufacturer recommendations and regulatory agency guideline
  • Assists in maintaining neat, orderly unit
  • Utilizes equipment time, and supplies economically
  • Maintains competency in operations, sanitizations, maintenance and troubleshooting of all equipment used for dialysis, water treatment and special procedures
  • Actively participates with the Unit manager in the care, routine cleaning and use of equipment
  • Reports all equipment and supply problems appropriately and according to policy to repair personnel/biomedical support staff
  • Records and reports all lab results as directed by the Unit Manager
  • Assists in orientation and training of new and existing staff
  • Acts as a resource person for staff members within the scope of practice
  • Assist in CQI projects with the healthcare team
  • Communicates effectively with physicians, supervisor, co-workers, patients and families
  • Maintains positive working relationship with Medical Director/Medical Staff
  • Actively participates in unit staff meetings and in services
  • Promotes supports and adheres to all policies outlined in facility policies
  • Completes clinical training program to successfully master ESRD patient care for hemodialysis, management skills, leadership development and gain confidence in listening and communication skills
  • Acquires information on the latest developments of dialysis technique by in-service classes
  • Performs additional duties and is responsible for specific projects assigned by Unit Manager

PHYSICAL DEMANDS AND WORKING CONDITIONS

  • The Physical demands and work environment characteristics described here are representative of those an employee encounters with performing the essential functions of this job.
  • This position provides direct patient care that regularly involves heavy lifting and moving of patients, and assisting with ambulation.
  • Ability to push, pull, mobilizes (with assistance of wheels, cars and other equipment) approximately 200 lbs.
  • Talking and listening to communicate ideas or requirements to individual staff members.
  • Visual acuity to prepare and read written material accurately.
  • Audio acuity to listen attentively and accurately.

 

EDUCATION AND/OR EXPERIENCE:

  • Graduate from an accredited school of nursing. Bachelors in nursing or health related field preferred.
  • One year nursing experience.
  • Hemodialysis experience desirable
  • ICU/CCU experience preferred.
  • Successful completion of training course in theory and practice of hemodialysis and peritoneal dialysis.
  • Successful completion of current CPR Healthcare Recognition according to the American Heart Association.

 

CERTIFICATES and/or LICENSE:

  • Current appropriate state license
  • Valid driver's license and insurance, when indicated/applicable for business travel by automobile.
  • Successful completion of current CPR Healthcare Recognition according to the American Heart Association.

 

KNOWLEDGE, SKILLS, and/or ABILITIES:

  • Ability to communicate clearly and concisely both orally and in writing
  • Ability to listen and respond in an understanding manner
  • Ability to reach adults and incorporate ethnic and spiritual needs of patients
  • Ability to foster an environment of cooperation, collaboration and teamwork
  • Computer knowledge desirable.
