SCHEDULE: PRN coverage of FT staff PTO.

Shift coverage includes 5:30am-1:30pm, 8:00am-4:00pm, or 10:00am-6:00pm. All Monday-Friday.

Ability to provide on-call coverage for nights, weekends, and holidays. 

• →Consult with medical staff regarding potential drug interactions, patient medical history, and use of particular medications.
• →Collaborate with medical staff to plan, monitor, review, and evaluate the quality safety and effectiveness of medication regimens.
• →Analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
• →Assess the identity, strength, or purity of medications. Compound and dispense medications as prescribed by physicians and other providers by calculating, weighing, measuring, and mixing ingredients – or overseeing these activities.
• →Ensure pharmacy data is recorded, maintained, and comply with regulations, security, and OSS Health procedures.
• →Provide direct clinical pharmacy services to patients, including medication therapy review, optimization, and monitoring.
• →Participate in medication management, including antimicrobial prophylaxis, venous thromboembolism (VTE) prophylaxis, analgesia, and anesthesia‑related pharmacotherapy.
• →Support evidence‑based pain management strategies, including multimodal analgesia, opioid stewardship, and management of acute and chronic pain.
• →Review, verify, and optimize medication orders for appropriateness, safety, dosing, and therapeutic efficacy.
• →Identify, prevent, and manage adverse drug events and medication‑related problems, including allergies, drug interactions, and contraindications.
• →Provide medication management support for patients, including preoperative medication reviews and postoperative follow‑up as appropriate.
• →Assist with anticoagulation management for patients including perioperative holding, bridging, and resumption of therapy.
• →Provide education to patients, caregivers, and staff regarding medication use, safety, and best practices.
• →Support formulary management and standardization of medication protocols and order sets.
• →Participate in medication safety initiatives, including contributing to medication safety programs, including event review, root cause analysis, and implementation of risk‑reduction strategies.
• →Support compliance with regulatory and accreditation standards (e.g., Joint Commission, CMS, state board of pharmacy).
• →Assist in development, review, and maintenance of medication‑related policies, procedures, and clinical guidelines relevant to orthopedic practice.
• →Serve as a clinical resource to physicians, advanced practice providers, nursing staff, and other healthcare professionals.
• →Participate in interdisciplinary rounds, care conferences, and performance improvement initiatives as appropriate.
• →Support transitions of care by facilitating accurate medication reconciliation at admission, transfer, and discharge.
• →Participate in training and mentorship of pharmacy staff, residents, students, or learners as applicable.
• →Maintain professional competence through continuing education and adherence to best practice standards.
• →Other duties as assigned.
