CalAIM Community Health Worker

Minimum Qualifications

• →High school diploma or equivalent; Associate’s or Bachelor’s degree preferred
• →Minimum of 2 years of experience in community health, social services, or related field
• →Strong understanding of community resources and social determinants of health
• →Excellent interpersonal, communication, and engagement skills
• →Proficiency in electronic health records and basic computer systems
• →Bilingual skills preferred (Spanish or other relevant languages)
• →Must be proficient in technology, including documentation systems, EHR platforms, and communication tools
• →Ability to document in real-time and manage multiple systems effectively

Work Requirements

• →Ability to travel extensively within assigned county
• →Must reside within assigned service area
• →Ability to work occasional evenings or weekends for community events
• →Ability to work in a hybrid, field-based environment

Additional Requirements

• →Possess a valid California Driver’s License (Class C minimum)
• →Maintain a personal, operable vehicle for daily business use
• →Maintain current liability insurance that meets California’s minimum legal requirements
• →Must pass a Motor Vehicle Report (MVR) background check prior to employment
