Role: Case Manager, FSP  Report to: Program Manager Program: 1303-FSP Department: Mental Health Location: Wellness Center - 345 S. San Pedro Street, Los Angeles, CA 90013 Schedule: Monday - Friday, 8am - 4:30pm Status: Full-Time, Non-Exempt (hourly), Non-Management Benefits: Medical, Dental, Vision, Retirement Planning (403b), etc. Opening: 1    Summary Full Service Partnership is a specialized mental health program that provides wraparound and individualized field-based services to individuals with complex needs, including serious mental illness, chronic health issues, substance use disorders and homelessness. The Senior Case Manager is responsible for leading a collaborative effort to house the most vulnerable and chronically homeless individuals in SPAs 4 and 5. With the support of and in partnership with the rest of the FSP team, the Senior Case Manager will ensure that FSP clients have a successful process for obtaining housing and accessing other supportive services. Essential Duties and Responsibilities: Provide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishment Identify appropriate permanent housing options for clients and support them through the process such as by conducting screening interviews, completing housing documentation, and coordinating clients’ move-in with the housing authority and property managers Perform initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing Encourage and promote an environment that is strength based to assist clients in meeting their individual goals Complete all documentation and maintain productivity expectations for billable time, according to standards and as they are required by our funder (the Department of Mental Health) Actively participate in staff meetings and trainings Assist in crisis interventions as needed, including participating in on call rotation calendar for overnight and weekend After Hours Emergency Line Qualifications: Bachelor’s degree in related field, or an equivalent combination of education and experience Three (3) years or more of Case Management experience At least one (1) year of experience working with adults with severe, persistent mental illness and co-ocurring substance use disorders, and with chronically homeless individuals Current, valid CA Driver’s license, reliable personal vehicle with auto insurance, and willingness to transport clients in personal vehicle Experience and familiarity with LA County resources, benefits, and services to help clients meet their case management needs  Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills Strong interpersonal and team building skills Knowledge of Public Housing Authority and Housings subsidy preferred (Section 8, Shelter Plus Care, VASH) Preferred  Qualifications: Bilingual Spanish Work Environment Combination of field and office environment Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs. On occasion walk or drive to different local sites Will necessitate working in busy and loud environments Will be exposed to elements like cold, heat, dust, noise and odor May need to bend, stoop, twist, and sit throughout the day
