Under the supervision of the System Director, Pharmacy Clinical Services and according to standard procedures, provides clinical pharmaceutical care; coordinates pharmaceutical care with trauma/surgical intensive care health care providers, practitioners, and educators to ensure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective medication therapy; optimize medication use through drug order review, therapeutic drug monitoring, participation in high-risk emergencies including resuscitation; supervises support personnel in all work areas assigned; precepts pharmacy residents and pharmacy students; participates in clinical research protocols and drug therapy research activities as applicable. The clinical pharmacy specialist provides for staff development activities and the coordination of staff operational activities within trauma/surgical intensive care; serves as educational and informational resource to physicians, medical residents, advanced practice providers, pharmacy staff, nursing staff, patients, and other allied health professionals; assists with planning, organizing, and implementation of clinical pharmacy programs across Premier Health; identifies processes and tools to guide the pharmacy staff in the utilization of the most appropriate and cost-effective medication therapy including drug preparation and procurement, ensuring high quality, safety, and positive patient outcomes.
Education
Minimum Level of Education Required: Bachelor's degree
Additional requirements:
* Type of degree: B.S.Pharm from an accredited college of pharmacy is required
Preferred educational qualifications: PharmD from an accredited college of pharmacy
Licensure/Certification/Registration
* Current licensure as a registered pharmacist by the State of Ohio Board of Pharmacy, or eligibility to become licensed within 60 days of employment start date
* Board-Certified Critical Care Pharmacy Specialist (BCCCP) or completed within the timelines defined by BPS to sit for examination
* Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) obtained within 90 days of employment start date

Minimum Level of Experience Required: 1 - 3 years of job-related experience

Preferred experience: Meets the preceptor criteria as outlined by ASHP
Other experience requirements:
* Completion of a post-graduate year 2 residency (PGY2) or fellowship required -or-
* Completion of a post-graduate year 1 residency (PGY1) plus minimum of 2 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring -or-
* Minimum of 3 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring
Knowledge/Skills
* Fluent in the use of drug information resources
* Ability to evaluate primary medical literature and develop evidence-based recommendations for treatment regimens
* Ability to perform Medication Use Evaluations (MUE) and submit recommendations for change within the health system
* Ability to provide pharmacokinetic and weight-based dosing
* Possess good interpersonal skills, excellent verbal and written communication skills, and problem- solving skills
* Ability to read and write printed material in English
* Ability to effectively perform patient interviews and medication counseling
* Ability to work well with other health care workers and perform well within a team
* Proficient with bedside aseptic technique
* Must be customer-service oriented
* Proficiency with computer keyboard operations
* Proficiency with Microsoft Office applications preferred

Essential Duties & Functions:
* Optimize pharmaceutical care through oral and/or written communication with providers, nurses, and other healthcare professionals
* Provide consultative services to providers, nurses, and other healthcare professionals to ensure optimal cost-effective pharmaceutical care to intensive care patients
* Participate in multidisciplinary patient care rounds at Miami Valley Hospital
* Participate in associated multi-disciplinary committees
* Participate in code pink or code blue in area of practice
* Flexibility in scheduling essential, with proper notification to meet departmental workloads
* Familiar with all duties and procedures performed by professional and technical staff
* Periodic weekend staffing required
* Contributes to the development, review, or improvement of clinical or operational policies/guidelines/protocols/order sets
* * Creates in-services or presentations to pharmacy staff/other health professionals at organization
* Assist in preventing adverse effects, reporting / investigating suspected or documented adverse drug reactions and/or medication errors
* Project management skills to begin, coordinate, and bring timely closure to assigned projects
* Ability to coordinate implementation of changes with pharmacy operations team
* Participation in the System Pharmacy & Therapeutics Committee as requested
* Participation in local, state, or national organizations
* Precept pharmacy students and residents as assigned

Other Duties & Functions:
* Dispense medication doses to patients if necessary
* Ability to restock medications in automated dispensing cabinets if necessary
* Deliver medications to patient care areas if necessary
* Monitor drug inventory levels to ensure a continuous supply of medications without stockpiling supplies
* Troubleshoot problems and/or do minor maintenance on all pharmacy equipment, printers, computers, and telecommunication equipment with adequate training in the pharmacy
* Must be familiar with pharmacy software
* Oversee pharmacy technicians’ and interns’ preparation of medication doses if necessary
* Must maintain departmental certification for compounding sterile products, if applicable
* Must use aseptic technique, wear appropriate attire, and follow standards in aseptic compounding
* Participation in publications and research
