About the Role

~1 min read

Join the Geriatric Outreach Service as an Allied Health Assistant and help improve care and transitions for older people across the Limestone Coast. Working as part of a multidisciplinary team alongside senior allied health and nursing clinicians, you will support older people experiencing dementia, cognitive decline and complex health or functional needs.

Under the direction of Allied Health Professionals, you will deliver delegated interventions, support functional independence and wellbeing, and assist with care coordination, appointments and communication with residential aged care facilities. You will also play an important role in supporting safe, coordinated transitions from hospital to residential aged care.

This is a Monday to Friday role with no after-hours requirements, with flexibility available around the days of the week worked and hours worked each day.

You are a compassionate and adaptable Allied Health Assistant who enjoys working collaboratively to support older people with complex needs. You bring strong communication and organisational skills, sound judgement and the ability to work effectively under the direction of Allied Health Professionals while contributing positively to a multidisciplinary team.

To succeed in the Allied Health Assistant role, you will:

  • Hold a Certificate IV in Allied Health Assistance, or equivalent.
  • Demonstrate an aptitude for working with older people with complex health, functional, cognitive and psychosocial needs, including people from culturally and linguistically diverse backgrounds.
  • Be able to work under general direction and direct or remote supervision from Allied Health Professionals.
  • Bring strong written and verbal communication, interpersonal, time management, organisational and problem-solving skills.
  • Be able to work in a physically demanding environment, including assisting with patient therapy and handling patient-related equipment and devices.

The Geriatric Outreach Service supports older people across the Limestone Coast who may experience cognitive decline, dementia, functional deterioration or complex care needs. Working across acute hospitals and residential aged care settings, the service supports timely specialist assessment, coordinated care and safe transitions between care environments.

The multidisciplinary GOS team works with patients, families, residential aged care facilities and health professionals to improve continuity of care and respond to the challenges associated with the region’s geographical spread, ageing population and increasing complexity of older people’s health needs.
