✓Conduct comprehensive case management assessments upon admission and at regular intervals.
✓Develop and implement individualized care plans in collaboration with the client and treatment team.
✓Coordinate access to social, financial, medical, legal, housing, and vocational resources to address social determinants of health.
✓Collaborates with Peer Support Specialists to work together in assisting patients in addressing their social determinants of health
✓Monitor progress toward care plan goals and update plans as needed.
✓Maintain ongoing communication with internal clinical team members and external service providers to ensure continuity of care.
✓Facilitate warm hand-offs and follow-ups to support treatment retention.
✓Provide psychoeducation and support on navigating systems such as Medicaid, legal aid, child welfare, etc.
✓Advocate for clients' needs while promoting autonomy and empowerment.
✓Maintain accurate, timely documentation in accordance with Medicaid, Medicare, and CARF standards (including contact logs, service plans, progress notes).
✓Participate in interdisciplinary team meetings, treatment planning, and staff development trainings.
✓Identify and address potential risks to client safety and report concerns according to protocol.
✓Safely transport patients to and from appointments, facilities, and designated locations using a personal vehicle while maintaining professionalism and patient confidentiality.
✓Case Management Certification or equivalent experience preferred.
✓At least one year of experience working with individuals with SUD and/or co-occurring disorders required.
✓Knowledge of trauma-informed care, harm reduction, and motivational interviewing principles.
✓Strong interpersonal, organizational, and advocacy skills.
✓Familiarity with state specific Medicaid documentation standards and local community resources.
✓Proficiency in EMR systems (Athena EMR experience preferred).
✓Must possess a valid driver’s license in good standing.
✓Must maintain an active personal automobile insurance policy that meets or exceeds state minimum coverage requirements and provide proof of insurance upon hire and as requested thereafter.
✓Ability to sit, stand, and walk for extended periods during patient sessions, meetings, and daily work activities.
✓Frequent use of hands and fingers for typing, writing, and handling clinical documentation and materials.
✓Ability to occasionally lift and carry materials (e.g., patient files, training supplies) weighing up to 25 pounds.
✓Ability to respond promptly to emergency situations, including providing assistance to patients in distress or crisis situations.
✓Sufficient visual and auditory abilities to assess patient needs, read documentation, and communicate effectively with patients, families, and team members.
✓Ability to navigate clinic spaces, including patient rooms, offices, and common areas.
✓Work may involve exposure to high-stress situations requiring emotional resilience, professionalism, and effective de-escalation skills.
