Telehealth Nurse Practitioner

• →Conduct Comprehensive Health Assessments through telehealth
• →Review medical history, medications, and preventive care needs
• →Document HCC (risk adjustment) findings accurately
• →Close HEDIS (quality measure) care gaps
• →Use ICD-10 and CPT II codes for documentation
• →Deliver clear care plans and follow-up guidance
• →Complete documentation on time

• Active Nurse Practitioner license in Washington
• Registered or enrolled with Washington Medicaid
• 1+ year Family Medicine or Internal Medicine experience
• Experience caring for Medicare and Medicaid populations
• Familiarity with HEDIS and risk adjustment workflows
• Available at least 24 hours per week

• Additional active state NP licenses
• Value-based care experience
• Annual Wellness Visit experience
• HCC risk adjustment documentation experience

• Minimum 24 hours per week
• Weekday daytime availability preferred
• Thursday and Friday availability especially helpful
• Fully remote telehealth care
