The Community Health Coordinator provides administrative and operational support to ensure the effective coordination and delivery of community health programs and services. This role manages program logistics, maintains accurate records, coordinates communications, supports scheduling and reporting activities, and serves as a key point of contact for internal teams, community partners, and program participants.
SALARY RANGE: $27.00 - $29.00/hour
- Coordinate participant enrollment, intake, and eligibility verification.
- Manage program scheduling, registrations, and participant communications.
- Maintain accurate records, data entry, and HIPAA compliance.
- Support Lifestyle Coaches with onboarding, materials, and program logistics.
- Verify insurance and assist with claims documentation.
- Track attendance, follow up with participants, and prepare program reports.
- Assist with administrative tasks to ensure successful program operations.
- Attend meetings, online webinars, and trainings.
- Performs other duties as assigned.
- Ability to sit for extended periods while performing administrative tasks, computer work, and documentation.
- Ability to use a computer, keyboard, telephone, and other standard office equipment regularly.
- Ability to communicate effectively in person, by phone, and through electronic communication.
- Ability to occasionally stand, walk, bend, reach, and move throughout the office or work environment.
- Ability to lift and carry light office materials, files, or supplies (typically up to 10--20 pounds).
- Ability to attend meetings, trainings, and community events that may require occasional travel or movement between locations.
- Requires visual acuity for reviewing documents, entering data, and working with electronic systems.
- Ability to perform repetitive hand and wrist motions associated with computer and administrative tasks.
- Must be able to maintain professionalism and focus while managing multiple priorities in a busy healthcare environment.
- Minimum of 1 year of experience working with teens
- Strong customer service skills and the ability to work with individuals at all employee levels is required.
- Strong problem-solving skills and the ability to multi-task is required.
- Must be able to work a varied schedule, with frequent evening and weekend work
- Bilingual (Spanish) a plus
