Responsibilities

As an RN Admissions Nurse, you'll help ensure patients and families receive the information, guidance, and care they need from the very beginning of their hospice journey.

Responsibilities include:

→Conducting pre-admission evaluations to determine eligibility and appropriateness for hospice services
→Educating patients and families about hospice care and available services
→Facilitating the admission process and obtaining informed consent
→Completing comprehensive nursing assessments
→Developing initial plans of care tailored to patient and family needs
→Providing interventions that enhance comfort, dignity, and quality of life
→Serving as a trusted resource and advocate for patients and their families

Graduate of an accredited Registered Nursing program
Current RN license in the state of practice
Current CPR certification
Hospice experience preferred (1+ year), but not required
Strong clinical assessment and critical-thinking skills
Ability to make independent decisions and prioritize patient needs
Exceptional communication and interpersonal skills
Commitment to providing compassionate, patient-centered care
Knowledge of quality assurance and performance improvement practices
