Physical Therapist

BAYADA Home Health Care is seeking a Full-Time Physical Therapist (PT) to provide one-on-one home health visits to adult and geriatric clients throughout Burlington County, NJ.

As a BAYADA Physical Therapist, you'll help clients regain strength, improve mobility, increase independence, and achieve their personal goals—all in the comfort of their own homes. • Provide one-on-one physical therapy visits to clients in their homes throughout Burlington County.
• Perform comprehensive physical therapy evaluations and assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Develop and implement individualized plans of care based on client needs, goals, and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, gait and balance training, mobility training, and other appropriate treatments.
• Help clients improve strength, mobility, safety, and functional independence in their home environment.
• Monitor client progress and adjust treatment plans as appropriate.
• Educate clients, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, safety strategies, and ongoing care.
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team.
• Complete timely and accurate clinical documentation using BAYADA's electronic medical record (EMR) system.

• Current New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Home health experience preferred, but not required.
• Strong clinical assessment, communication, interpersonal, and organizational skills.
• Ability to work independently and effectively manage time and schedule.
• Prior experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel to client homes throughout Burlington County.

Base Pay, depending upon qualifications, status, and subject to negotiation:

$63.00-$73.00 per point
