Physical Therapist

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem opportunity with our South Jersey Senior Living office, providing one-on-one therapy visits to adult and geriatric clients living in facilities throughout Burlington and Camden Counties, NJ.

As a BAYADA Physical Therapist, you'll provide individualized therapy that helps clients improve mobility, strength, balance, safety, and overall functional independence—all within the place they call home.

• Provide one-on-one physical therapy visits to clients in facilities throughout Burlington and Camden Counties.

• Perform comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.

• Develop and implement individualized plans of care based on client needs, goals, and physician orders.

• Provide skilled physical therapy interventions, including therapeutic exercise, gait and balance training, mobility training, transfer training, and other appropriate treatments.

• Help clients improve or maintain strength, mobility, safety, and functional independence.

• Monitor client progress and adjust treatment plans as appropriate.

• Educate clients, families, caregivers, and facility staff on therapeutic exercises, mobility techniques, fall prevention, and safety strategies.

• Collaborate with nurses, therapists, physicians, facility staff, and other members of the interdisciplinary care team.

• Complete timely and accurate clinical documentation using BAYADA's electronic medical record (EMR) system.

• Current New Jersey Physical Therapist (PT) license.

• Graduate of an accredited Physical Therapy program.

• Minimum of one year of recent experience as a Physical Therapist.

• Home health or senior living experience preferred, but not required.

• Strong clinical assessment, communication, interpersonal, and organizational skills.

• Ability to work independently and effectively manage time and schedule.

• Prior experience with electronic medical records (EMR) preferred.

• Reliable transportation and ability to travel to facilities throughout Burlington and Camden Counties.

Senior living home health combines the individualized care of home health with the convenience of seeing clients within community-based settings. You'll have the opportunity to focus on one client at a time while working alongside facility staff and BAYADA's interdisciplinary clinical team.

You'll use your clinical expertise to help clients maintain their mobility, safety, and independence while providing care in a setting where you can build meaningful relationships with the clients and communities you serve.
