~2 min readThe Behavioral Health Care Manager (BHCM) is a core member of the Collaborative Care (CoCM) team and is responsible for coordinating and delivering evidence-based, measurement-based behavioral health services within primary care. The BHCM partners closely with primary care providers, psychiatric consultants, and interdisciplinary teams to support patients with common mental health and substance use disorders.
This role emphasizes population-based care, systematic follow-up, registry-driven tracking, and brief interventions aligned with Collaborative Care Model standards. Responsibilities include:
- →Support the mental and physical health care needs of patients on an assigned caseload.
- →Coordinate care closely with medical providers and other mental health providers as appropriate.
- →Screen and assess patients for common mental health and substance abuse disorders.
- →Facilitate patient engagement, follow-up care, and provide education on treatment options.
- →Track treatment response and monitor symptoms, side effects, and treatment adherence in person or by phone.
- →Support psychotropic medication management as prescribed by medical providers.
- →Provide brief evidence-based behavioral interventions, including behavioral activation, Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT)-informed skills, motivational interviewing, and solution-focused interventions.
- →Provide or coordinate referrals to evidence-based psychosocial treatments and outside services, including specialty mental health care, substance abuse treatment, social services, and vocational rehabilitation.
- →Participate in regular caseload consultations with psychiatric consultants and communicate treatment recommendations to medical providers.
- →Track patient follow-up and outcomes using a registry/EHR system, documenting encounters, progress, and treatment recommendations.
- →Facilitate treatment plan changes for patients not improving as expected in collaboration with providers and psychiatric consultants.
- →Develop relapse prevention and self-management plans for patients preparing for discharge from the caseload.
- →Other duties as assigned.
- Master’s-level degree in a behavioral health field required.
- Mental Health Licensure or Associate level licensure (e.g., LCSW(A), LCMHC(A), LMFT(A)) preferred.
- Experience in integrated care, primary care, or collaborative care settings preferred
- Experience with screening for common mental health and/or substance abuse disorders.
- Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role.
- Experience with evidence-based counseling techniques
