On behalf of our client, Peoria-based Active Vital Care, we seek a compassionate, patient-focused Podiatrist to provide initial and urgent visits to patients in private homes, group homes, independent, assisted and skilled living facilities. They may also see patients in our Chronic Care Management Centers in person or via Telemedicine visits. The Podiatrist will be responsible for conducting in-home visits in an assigned geographic area of responsibility. 

  • Perform professional health services for patients. 
  • Evaluate, update, and prescribe medications and collaborate with other members of the treatment team to monitor medications and facilitate coordination between all specialties. 
  • Examining a patient’s leg, ankles, and feet to diagnose diseases, illnesses, or injuries.
  • Performing X-rays and physical scans of a patient’s lower extremities.
  • Advising patients on proper foot care and therapeutic techniques
  • Prescribing orthotics and pain relief medication.
  • Analyze and evaluate patient data or test findings to make diagnoses and/or develop differential diagnosis. Performs reviews of charts and treatment plans. 
  • Design individualized care plans, using a variety of treatments and collaborate in the development of individualized habilitation or treatment plans. 
  • Provide coordination of services with the patient's primary care physician to design treatment plans and monitor progress. 
  • Consult with patients and/or POAs at home/communities and clinic to determine the appropriate course of treatment. 
  • Clearly and effectively communicate with PCP and to coordinate with PCP, patient, and POA.

  • License must be current and in good standing in the State of Arizona. If not licensed, be willing to get licensed. 
  • Board eligible/certification in Podiatry. 
  • Two to four years experience as a Podiatrist is preferred.
  • The successful candidate will also possess excellent clinical, teaching, interpersonal skills, and ability to participate in the development of skills including planning, implementation, and evaluation of outcomes. 
  • Competency and experience in the area of Podiatry 
  • Proficient on a computer, laptop, tablet and smartphone 
  • Ability to read, write, and communicate effectively 
  • Must have current DEA license
