ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled Private Duty Nurse (PDN) to join our dedicated team in San Diego, CA. This is a rewarding opportunity to make a meaningful difference in the lives of patients by delivering high-quality, one-on-one nursing care in the comfort of their own homes.

Responsibilities:

Provide direct private duty nursing care to patients in a home health setting
Assess, monitor, and document patient health status and progress
Administer medications, treatments, and medical procedures as prescribed
Develop and implement individualized patient care plans
Communicate effectively with patients, families, and the care team regarding patient needs and updates
Educate patients and caregivers on health management, medications, and safety
Maintain accurate and timely clinical documentation in compliance with regulations

Requirements:

Active LVN license in the state of California
Prior experience in private duty nursing, home health, or pediatric care preferred
Current CPR/BLS certification required
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently and manage time effectively in a home setting
Reliable transportation and valid California driver's license
