Director of Nursing & Clinical Operations (DNCO) – Hospice

Full-Time | Home & Facility-Based Care
Location: Arlington, TX + Service Area (DFW Metroplex)

Lead from the Field. Build with Purpose.

At Hospice Care Partners, we are not looking for a Director of Nursing who sits behind a desk.

We are looking for a hands-on clinical leader who is visible in the field—supporting nurses, strengthening facility relationships, and ensuring that patient care is delivered at the highest level.

As we grow, this role will be central to building a hospice team known for:

  • Proactive care—not reactive
  • Clear, consistent communication
  • High accountability across every level of the team

If you are looking for a traditional, office-based DON role, this is not the right fit.

If you lead from the front, hold high standards, and take ownership of outcomes—we want to meet you.

Role Overview

The Director of Nursing & Clinical Operations (DNCO) is a Registered Nurse responsible for the overall leadership, performance, and quality of clinical services across both home and facility-based care.

This is a field-engaged leadership role, combining:

  • Clinical oversight
  • Team development
  • Operational execution
  • Relationship management

You will lead a multidisciplinary team including:

  • RN Case Managers
  • Hospice Aides
  • Social Workers
  • Chaplain/Bereavement Coordinators
  • Volunteers

You may also maintain a limited caseload as needed.

What You’ll Do

  • Lead and manage all clinical operations across home and facility settings
  • Be present and visible in the field:
    • Shadow RN visits
    • Support staff in real-time patient care situations
    • Engage directly with facility partners and referral sources
  • Set and enforce standards for communication, accountability, and care delivery
  • Ensure appropriate staffing, productivity, and case distribution
  • Coach, develop, and hold clinical staff accountable to expectations
  • Address performance issues early and directly
  • Coordinate care across interdisciplinary teams, patients, families, and referral partners
  • Monitor, analyze, and act on QAPI data to improve outcomes
  • Ensure compliance with hospice regulations, documentation, and best practices
  • Strengthen clinical performance in key facility partnerships (e.g., nursing homes)

What Makes This Role Different

  • You are not managing from a distance—you are leading in the field
  • You will directly influence patient experience, team performance, and referral relationships
  • This is a build-and-scale role, not a maintenance position
  • You will help define the standard of care and communication for the organization

Who You Are

  • A strong clinical leader who leads by example—not just direction
  • A proactive operator who solves problems before they escalate
  • A confident communicator who can navigate difficult conversations with staff, families, and partners
  • A decisive leader who addresses underperformance directly
  • A team builder who creates accountability, trust, and consistency
  • A mission-driven professional who balances clinical excellence with compassion

Minimum Requirements

  • Current, unencumbered Texas RN License
  • 3+ years hospice case management experience
  • 1+ years of hospice leadership/management experience
  • Reliable transportation with current insurance
  • Current CPR certification
  • Current TB test (negative) or equivalent
  • Ability to travel throughout service area

Preferred (Not Required)

  • Experience scaling or growing a hospice program
  • Strong QAPI and clinical performance improvement experience
  • Experience leading interdisciplinary teams
  • Familiarity with Consolo EMR

Non-Negotiables

  • You take ownership of clinical outcomes and team performance
  • You are visible, engaged, and present in the field
  • You hold people accountable—consistently and professionally
  • You communicate clearly and proactively
  • You address issues early instead of allowing them to escalate

Compensation

  • Base Salary: $95,000 – $125,000+ (based on experience)
  • Performance Bonuses: Monthly and quarterly incentives tied to:
    • Census growth
    • Clinical quality
    • Team performance and retention
  • Auto Allowance
  • Phone Allowance
  • Health Insurance Benefit Allowance

(We will structure compensation competitively for the right leader.)

What We Offer

  • Clinician-led organization with direct access to leadership
  • Opportunity to build and shape a high-performance clinical team
  • A role with real visibility, influence, and impact
  • A mission-driven environment where your leadership directly affects patient and family experience

Final Thought

The best clinical leaders don’t sit behind desks—they show up when it matters most.

If you’re the kind of leader who leads from the field, supports your team in real time, and takes pride in building something exceptional—we’d like to speak with you.
