Responsibilities
~1 min readTo be successful in this role, you will
- →Complete home assessments across South Oxfordshire (own transport required)
- →Conduct bedside assessments in hospital settings
- →Carry out Care Act assessments and develop person-centred plans
- →Promote independence and support individuals on their reablement journey
- →Work both independently and as part of a team to deliver high-quality care
Requirements
~1 min read- Minimum 2 years post-qualified
- Strong assessment and organisational skills
- Knowledge of relevant legislation and reablement practices
- Must have own transport
- Experience in promoting independence and delivering community-based care
