Responsibilities

- →Safeguarding: Identify and respond to safeguarding concerns, conducting investigations and implementing protection plans to ensure the safety of vulnerable adults with disabilities.
- →Mental Capacity Assessments: Conduct assessments to determine the mental capacity of adults with disabilities, ensuring decisions are made in their best interests.
- →Multi-Disciplinary Collaboration: Collaborate with a range of professionals, including healthcare providers, therapists, and community organizations, to provide holistic support to adults with disabilities.
- →Advocacy: Advocate for the rights and needs of adults with disabilities, ensuring their voices are heard and their preferences are respected in care planning and decision-making processes.
- →Crisis Intervention: Provide immediate support and intervention in crisis situations, working to stabilize and support adults with disabilities and their families.
- →Record Keeping: Maintain accurate and up-to-date records of assessments, care plans, and interventions, ensuring compliance with legal and organizational standards.
- →Social Care Assessments: Conduct comprehensive social care assessments to identify the needs and strengths of adults with disabilities, ensuring their well-being and safety.
- →Care Planning: Develop, implement, and review personalized care plans in collaboration with adults with disabilities, their families, and other professionals to meet their individual needs.

~1 min read- Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
- Registration: Must be registered with Social Care Wales, the regulatory body for social workers in Wales.
- Experience: Significant experience in delivering social work services to vulnerable adults, including those with disabilities and complex needs.
- Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Social Services and Well-being (Wales) Act 2014 and the Mental Capacity Act 2005.
- Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
- Driving Licence: A full UK driving licence and access to a car are often required for client visits.
