Social Worker - Community Team - Cheshire East

Liquid Personnel is currently recruiting for a Social Worker to join a Community Team based in Cheshire East.

• →Conduct comprehensive assessments to determine the needs and risks of individuals.
• →Develop and implement care plans in collaboration with service users, families, and multidisciplinary team's
• →Provide guidance and support to individuals, empowering them to access services and maintain independence.
• →Advocate on behalf of service users to ensure their rights and needs are met.
• →Maintain accurate records in compliance with local policies and legislation.
• →Work closely with healthcare professionals, voluntary organizations, and community groups to coordinate care.
• →Ensure safeguarding principles are upheld, identifying and responding to concerns appropriately.
