Social Worker - Learning Disabilities Team

Liquid Personnel is recruiting a Qualified Social Worker for its client’s Learning Disabilities Team at the London Borough of Tower Hamlets.

This is a 6-month post aimed at addressing the backlog of Care Act reviews within the Community Learning Disability Service—currently totaling 475 outstanding reviews across three teams. This is a great opportunity for experienced professionals to make a meaningful impact in a high-demand area.

• →Conduct comprehensive assessments of individuals with learning disabilities to determine their needs and eligibility for services.
• →Develop and implement personalized care plans in collaboration with service users, their families, and other professionals.
• →Prioritize and manage a caseload of individuals on the waiting list, ensuring timely interventions and support.
• →Work collaboratively with healthcare providers, educational institutions, and community organizations to coordinate services and resources.
• →Monitor and review care plans regularly to ensure they meet the evolving needs of service users.
• →Respond to safeguarding concerns and take appropriate action to protect vulnerable individuals.
• →Maintain accurate and up-to-date records of all interactions and interventions.
• →Provide emotional and practical support to service users and their families.
• →Advocate for the rights and needs of individuals with learning disabilities.
• →Participate in regular training and professional development activities.

To be successful in this role you must have, 

• Bachelor's degree in Social Work or a related field.
• Registration with the Health and Care Professions Council (HCPC).
• Experience in working with individuals with learning disabilities.
• Strong understanding of relevant legislation and policies, including the Care Act 2014.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.
• Empathy, patience, and a commitment to supporting individuals with learning disabilities.
