As an RN Clinical Coordinator, you’ll play a vital role in bringing that mission to life.

Schedule: Exempt (salaried), min 40 hours/week | Monday - Friday | Generally 8:00 a.m. to 4:30 p.m, with some flexibility depending on the needs of the day

What You’ll Do:

Join a mission-driven team where you’ll deliver compassionate, patient-centered care across home, clinic, and partner facilities. As an RN Clinical Coordinator (Registered Nurse), you’ll take the lead in managing complex care using PACE-specific assessments, collaborating with a dedicated Interdisciplinary Team to create and execute personalized care plans.
