Clinical Social Worker, MSW

As our Clinical Social Worker, you'll provide psychosocial assessment, counseling, care coordination, and advocacy for residents and their families across our full continuum of care: Independent Living, Assisted Living, Memory Care, and hospice. You'll support emotional well-being and work with our interdisciplinary team to keep care centered on each person.

• →Build and keep a relationship with each resident, and support their families as needed
• →Keep regular office hours and drop-in times so residents and families can easily reach you
• →Complete psychosocial assessments after move-in, with annual check-ins
• →Identify residents' emotional, social, and environmental needs, and put support plans in place
• →Help residents adjust to aging, community living, chronic illness, and changes in health, and find purpose in their new home
• →Offer education, support, and resources to residents, families, and staff on dementia, end of life and loss, anxiety, and depression
• →Share information on resident rights, living wills, power of attorney, and advance directives on request
• →Work with residents, families, and the Care Coordination Team so moves between levels of care go smoothly

Required

• Master's degree in Social Work (MSW) from a CSWE-accredited program
• Current New Hampshire social work license
• At least two years of social work experience in health care, long-term care, rehabilitation, hospice, or another setting serving older adults
• Skills in psychosocial assessment, counseling, crisis intervention, and care planning
• Knowledge of aging, dementia, long-term care, and healthcare systems
• Strong written, verbal, and interpersonal skills, including conflict management
• Comfort with Microsoft Office and electronic health record software (PointClickCare or similar)

Preferred

• Coursework or specialized training in gerontology or palliative/hospice care
• Experience with older adults across Independent Living, Assisted Living, and Memory Care
• Certification in gerontological social work (ASW-G) or hospice and palliative care (APHSW-C)
