Internal Hire - Housing Case Manager

The St. Francis Center (SFC), a non-profit institution of the Episcopal Diocese of Colorado, was established in June 1983. SFC is a refuge for homeless people in the metro Denver area. Our programs and services are tailored toward adults, and focus on daytime shelter and basic needs, non-congregate shelter, social services, health and wellness, housing, employment, and outreach. At SFC, “guests” and tenants in our housing programs can work with a variety of specialists, depending on their individual needs, which is often the first step toward transformation and ultimately self-sufficiency.
We are currently looking for Housing Case Manager. This is a Full-Time, 40 hours/week, non-exempt position eligible for an excellent benefit package as outlined below.
***This position is a 3pm – 11pm shift and may include coverage of other shifts on weekdays, and weekends.
POSITION OVERVIEW
The team provides comprehensive case management services to people who are transitioning out of homelessness who experience a disabling condition(s) such as a mental illness, disability, medical challenges and/or substance misuse. SFC works collaboratively with other service providers in the community to provide optimum coordination, communication and continuity of services for tenants and potential residents of the housing program.
Job Responsibilities
- Provide clinical case management and on-going support for previously homeless housing residents in independent living apartments who have a history of trauma, and live with chronic mental illness, chronic health conditions, physical disability and/or history of substance abuse.
- Develop and carry out appropriate individualized housing stability plans based on identified goals along with documented case management protocols to ensure support for the tenant to achieve their goals and stabilizes in housing.
- Maintain timely, accurate resident records and departmental documentation.
- Establish supportive and effective relationships by listening with care, treating residents with respect and dignity, and responding to each individual’s unique needs, while maintaining clear professional boundaries and confidentiality.
- Work with people from a variety of racial, cultural, social and economic backgrounds with various lifestyles, sexual orientations, ages, genders and disability status including current or past substance use and mental health challenges.
- Collaborate and advocate on behalf of resident needs across many systems including medical, mental health, legal, educational, employment, public benefits and community service providers.
- Additional client centered duties include ongoing meetings with residents, crisis intervention, securing benefits and resources, providing transportation, problem solving, medication management/monitoring (if needed), conflict management and de-escalation, budgeting/money management
- Collaboration with property management is required to assist tenants in maintaining stability while taking into account the needs of other tenants and safety of everyone in the building.
- Planning and engaging in activities and community building with residents and other staff.
- May require other shift hours to support resident activities and wellbeing.
- Other duties assigned may include stepping in for other staff positions when the need arises or other assignments.
Requirements/Qualifications:
- BA or BS with experience 1 year assisting people living in poverty or who have experienced homelessness; alternate degree with minimum 2 years’ experience will be considered.
- Ability to connect with groups of residents, engage them with one another, create, schedule and implement activities programing and essential resource access for residents. Strong organizational ability and the capacity to set and hold positive behavioral expectations for residents.
- Knowledge of a strengths based, person centered approach, Housing First principals, Trauma Informed Practices, crisis intervention, brief and long-term mental health therapy modalities, motivational interviewing, and the principles of harm reduction.
- Understanding of and experience with addressing the impact of trauma, chronic medical and mental health conditions, disabilities and aging
- Strong verbal and written communication skills.
- Excellent computer skills including Microsoft Office suite; HMIS and Therapy Notes programs. Timely and accurate documentation of services provided is required and monitored.
- Demonstrated efforts in professional development and positive response to feedback and clinical supervision.
Compensation and Benefits:
$23-26/hour or DOE
This is a full-time, 40 hours/week, non-exempt position
