Job Description

St. Francis Center (SFC), an institution of the Episcopal Diocese of Colorado, was established in June 1983. SFC is a refuge for people who are experiencing homelessness in the metro Denver area. Our programs and services are tailored toward adults and focus on daytime shelter, social services, health and wellness, housing, employment, and outreach. At SFC, “guests” have the opportunity to work with a variety of specialists, depending on their individual needs, which is often the first step toward transformation and ultimately, self-sufficiency.  

We are currently hiring transfer applicants only for a Case Manager for our 24-hour, Non-Congregant Shelter. This is a Full-Time, non-exempt position eligible for an excellent benefit package as outlined below.  


POSITION OVERVIEW

The Case Manager position, based at the Non-Congregate Shelter (NCS), will coordinate services for guests while fostering collaboration with community partners and service providers.

Job Responsibilities (duties include but are not limited to):

  • Engage and develop rapport with guests at the NCS – Stone Creek
  • Stabilize and prepare for permanent housing
  • Assess the level of need of guests
  • Assist guests in accessing cash and non-cash benefits, referrals, and community resources that help identify and address barriers to housing, creating a pathway out of homelessness.
  • Support guests in obtaining vital documents
  • Connect guests with services to address mental health and addiction, and medical needs.
  • Complete timely data entry in HMIS, including tracking goals, One Home assessments and reassessments, outcomes, referrals, and other relevant information as required.
  • Accompany guests to community appointments, providing support and advocacy at the Department of Motor Vehicles, Social Security Office, Denver Housing Authority, and with other community partners.

Qualifications & Requirements

  • Education & Experience
    • Bachelor’s degree in social work, human services, psychology, or related field preferred. Training certificates in trauma-responsive care, person-centered care, co-occurring disorders in addiction and mental illness, HMIS, and One Home are preferred. 
    • Minimum of 2 years of experience providing direct services to individuals experiencing homelessness, housing instability, mental health, or substance use challenges.
    • Experience working with diverse populations and individuals with complex barriers to housing.
  • Skills & Competencies
    • Strong interpersonal skills with the ability to build rapport and maintain professional boundaries.
    • Knowledge of trauma-responsiveness, harm reduction, person-centered approaches, and housing first principles.
    • Ability to conduct intakes, assess needs, and develop individualized service plans.
    • Proficient in data entry and case documentation, preferably in HMIS or similar databases.
    • Experience with community resources, housing programs, and public benefit systems (e.g., SNAP, Medicaid, SSI/SSDI, housing vouchers).
    • Strong organizational and time-management skills, with the ability to manage multiple priorities.
  • Other Requirements
    • Ability to accompany guests to community appointments, which may include driving or using public transportation.
    • Background checks are required, and compliance with agency policies regarding confidentiality and professional conduct is mandatory.
    • Ability to commit to working 1 weekend day per month.
    • Valid driver’s license and reliable transportation required. 
    • Basic computer skills are necessary, and knowledge of Microsoft Office programs is preferred.
    • Willingness and ability to work with people from a variety of racial, cultural, social, and economic backgrounds with various lifestyles, sexual orientations, and of all ages and genders

  • Ability to communicate with written correspondence and verbal interactions to help encourage and motivate guests in their work to regain community housing and stability.
