We are seeking a clinically-informed, operationally-minded Senior Manager to own the end-to-end design and implementation of care models for Twin's most strategic clients. Reporting to the Senior Director of Clinical Performance and Innovation, this role is responsible for bringing new care models to life, from early discovery through pilot, launch, and steady-state performance management. These programs will be built with the member and client in mind, including health systems, integrated delivery networks, Medicare Advantage plans, and new product lines. You will be the connective tissue across the organization, corralling clinical operations, implementation, product, technology, finance, and senior leadership around a shared vision of success for each program.
Responsibilities
~2 min read- →Own end-to-end program design and operational strategy for new care model implementations, from discovery and design through pilot, launch, and post-launch management. These programs span new client partnerships, new program types (e.g., hypertension), and adaptations of existing care models.
- →Design operationally sound care models for new clients by understanding their member population, existing care infrastructure, and strategic priorities. Translate that understanding into scalable workflow design, care team structures, member engagement strategies, and success metrics.
- →Manage operational implementation in real time: anticipate operational bottlenecks, diagnose issues as they emerge during pilot and launch, and drive solutions. This requires hands-on problem-solving, not step-back oversight.
- →Lead cross-functional collaboration across in partnership with clinical operations, implementation, product, technology, finance, and senior leadership to align on implementation approach, resolve tradeoffs, and keep initiatives moving forward.
- →Serve as a primary internal point of contact for strategic client partners, including health systems, integrated delivery networks, and Medicare Advantage plans, during implementation and steady-state operations. Build trusted relationships with client stakeholders, communicate progress and performance transparently, and surface and resolve issues before they escalate.
- →Develop and maintain operational frameworks, playbooks, and enablement materials that translate care model design into day-to-day practice for care teams, including routing logic, member prioritization, escalation pathways, and cross-functional handoffs.
- →In alignment with senior leadership, define success metrics for each implementation (e.g. clinical outcomes, operational efficiency, member satisfaction, financial performance) and build measurement infrastructure to track progress and accountability.
- →Manage up and across: communicate regularly with senior leadership on progress, risks, and strategic tradeoffs. Escalate issues with clarity and candor. Partner with peer leaders to solve problems that span functions.
- →For strategic client relationships, continue to own operational strategy and performance management into steady state, acting as the internal strategic operator for that partnership.
- →Contribute to the overall care model roadmap and product strategy by synthesizing implementation learnings, clinical insights, and client feedback into opportunities for improvement and scale.

Requirements
~2 min read- 7–10 years of healthcare experience, with at least 4 years in clinical operations, care delivery, or a closely related operational role.
- 2–3 years of experience in a strategic operations, clinical innovation, or internal consulting role where you've been tasked with designing and implementing new programs or processes across multiple functions.
- Demonstrated track record of working directly with clinical teams, physicians, and other clinical leaders to design and implement operational or care model changes.
- Hands-on experience implementing complex, multi-stakeholder initiatives in a fast-moving environment. You're comfortable with ambiguity, can define your own scope, and drive work forward without needing everything spelled out upfront.
- Strong analytical and structured problem-solving mindset. You can do first-pass data analysis to diagnose problems, form hypotheses, and evaluate solutions.
- Comfort with healthcare vocabulary and clinical concepts. You're a quick learner who can dive into the details of a new clinical area with subject matter experts and ask smart questions.
- Excellent written and verbal communication skills. You can write crisp operational specs and implementation plans, distill complex initiatives into executive-ready summaries, and develop clear guidance for care teams.
- Ability to navigate matrix dynamics and manage both up and across. You're comfortable with ambiguity around decision rights and can build alignment without formal authority.
- Startup experience or startup mindset — you've worked in a scrappy, fast-moving environment where you wore multiple hats and adapted quickly to changing priorities.
- Proficiency with Google Suite applications.
- Willingness to travel occasionally for client on-site work and implementation support.

Preferred
- Direct experience implementing new care models or service lines in a digital health, virtual care, or telehealth environment.
- Experience working with Medicare Advantage plans, health systems, or integrated delivery networks.
- Background in value-based care, accountable care organizations, or similar risk-bearing care models.
- Consulting background (management consulting, healthcare consulting, or internal strategic operations role at a larger healthcare organization).
- Experience with healthcare data tools, reporting dashboards, or member engagement platforms.
- Master's degree in health administration, public health, business administration, or related field.
- Prior experience working on product development, roadmapping, or cross-functional product initiatives.
What We Offer
