Community Health Worker Lead

  How This Role Contributes to the Y’s Mission As a Community Health Worker, you will help individuals and families overcome barriers to better health by connecting them with healthcare, food, housing, transportation, insurance, and other community resources. You will conduct outreach, manage a caseload, support food-access and chronic-disease prevention programs, and help community members take meaningful steps toward their health and wellness goals. Essential Functions Conduct community outreach and connect individuals with appropriate health and social-service resources. Use an approved screening tool to identify needs related to food, housing, transportation, healthcare, and economic stability. Provide warm handoffs to service providers and document all referrals and follow-up activities. Manage a caseload of community members participating in health and wellness programs. Help clients develop goals, understand care plans, manage chronic conditions, and overcome barriers to care. Coordinate communication among clients, healthcare providers, community organizations, and internal teams. Maintain accurate electronic client records, case notes, and monthly reports. Support evidence-based chronic-disease prevention programs. Serve at Y food pantries and support Y Fresh Mart and other food-access initiatives. Participate in Y on the Fly activities, quarterly Community Health events, and West Baltimore outreach events. Travel to client homes, clinics, Y locations, agencies, and community events. Schedule and Work Requirements Regular work hours are 10:00 a.m.–6:00 p.m. The position requires working three (3) Saturdays each month. Frequent local travel is required. Ability to maintain confidential information, professional boundaries, and accurate electronic documentation.
