One Health currently operates 35 primary care offices across the greater Charlotte and Winston-Salem areas of North Carolina. Our specialties include Family Medicine, Internal Medicine, Pediatrics, Sports Medicine, Rheumatology, OB/GYN, and Urgent Care.

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our expanding Transitional Care Management Team. This role focuses on caring for high-risk patients recently discharged from the hospital, with the goal of improving outcomes, reducing readmissions, and ensuring a smooth transition back to the community.

• →Provide Transitional Care Management services to recently discharged patients.
• →Collaborate closely with hospitalists, Emergency Department providers, and Case Management teams at Atrium Health Lake Norman Hospital.
• →Work as part of a multidisciplinary team including Registered Nurses, LPNs, Pharmacists, and Social Workers.
• →Office hours are Monday through Friday.
• →Access additional system-level care management resources and virtual technology.

• Experienced in hospital-based or Emergency Department settings (preferred).
