Responsibilities

→Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practices
→Monitor patient progress and adjust care plans to achieve optimal outcomes
→Provide clear, timely, and compliant documentation to support care and reimbursement
→Educate patients and families on treatment plans, self-care, and safety practices
→Coordinate care with physicians, nurses, and other healthcare team members
→Participate in case conferences, team meetings, and quality improvement initiatives
→Support safe patient handling and promote fall and injury prevention

Strong clinical judgment and ability to manage a diverse patient population, including complex cases
Excellent communication and collaboration skills
Ability to work independently and manage time effectively in a field-based role
Commitment to delivering compassionate, patient-focused care

Requirements

Licensed Physical Therapist in Illinois (or eligible for licensure)
Minimum 1 year of clinical experience (or strong clinical affiliation experience)
Basic computer skills (documentation systems, email, etc.)
Valid driver’s license and reliable transportation

Travel required within assigned service territory
Work in patient homes and assisted living settings
Ability to lift up to 35 lbs (with assistance/equipment as needed)
Frequent standing, walking, bending, and assisting patients with mobility and transfers
