• →Provide physical therapy services according to a physician’s plan of care and state practice guidelines
• →Evaluate patient function and assist in developing individualized care plans
• →Educate patients, families, and caregivers on treatment plans and therapeutic activities
• →Translate prescribed exercises into functional activities and activities of daily living
• →Recommend modifications to equipment and home environments to support patient safety and independence
• →Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress
• →Document clinical notes, progress reports, and summaries in a timely and accurate manner
• →Support and motivate patients and families throughout the rehabilitation process
• →Supervise Physical Therapist Assistants in accordance with company policy and state regulations
• →Maintain accurate clinical records and track patient goals and outcomes

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
• Current Physical Therapist (PT) license in the state of practice
• At least two (2) years of experience in an acute care or similar setting (preferred)
• Current CPR and First Aid certification
• Reliable transportation, valid driver’s license, and current automobile insurance
