Responsibilities

→Evaluate patients and develop individualized treatment plans based on physician orders.
→Provide skilled physical therapy services in the patient’s home.
→Help patients improve mobility, strength, balance, and overall functional independence.
→Educate patients, families, and caregivers on treatment plans, home exercises, and safety strategies.
→Recommend adaptive equipment and home modifications to promote safety and independence.
→Collaborate with physicians and the interdisciplinary care team to coordinate patient care.
→Accurately document evaluations, progress notes, and patient outcomes in a timely manner.
→Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.
→Encourage and support patients throughout their rehabilitation journey.

Requirements

Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
Current Physical Therapist (PT) license in the state of practice.
Home Health experience preferred, but not required.
Current CPR certification.
Valid driver’s license, reliable transportation, and current automobile insurance.
