Completes comprehensive individualized client assessments, including relevant medical history and diabetes-related knowledge, skills, and behaviors. Develops and implements education plans with specific learning objectives tailored to individual client needs. Teaches clients in both group and individual settings using a variety of teaching techniques and tools. Evaluates client progress towards achieving learning objectives and behavior change goals. Assists clients in identifying effective behavior changes and setting achievable goals for diabetes self-management. Coaches clients through the behavior change process to support effective diabetes self-management. Completes accurate and timely documentation of client encounters and communicates progress to referring physicians. Collaborates with multidisciplinary team members to meet assessed client needs. Manages a multiple-client caseload while maintaining confidentiality and discretion. Evaluates clients' glycemic control using lab results and self-monitored glucose tests, recommending behavior changes as needed. Maintains CDE and BLS certifications and enhances professional growth through educational programs and literature review. Promotes the education program to associates, physicians, and the community, and participates in continuous quality improvement goals and outcome measurement. Performs other duties as assigned.
