Schedule: Full Time Nights

Shift:  6:45pm to 7:00am with every other weekend. 

Location:  Covering Orange, Osceola and Seminole Counties. 

• Provides skilled nursing care to hospice patients in private homes, skilled nursing facilities, assisted living facilities, and inpatient units.

• Assesses patient conditions and symptoms to determine the need for crisis care.

• Administers medication, takes vitals, feeds, bathes, changes clothes and bedding, dresses wounds, and provides other palliative care as indicated in the care plan.

• Offers support and education to the patient's family and caregivers.

• Communicates with supervisory RN regarding symptom management, changes in patient condition, safety issues, and other patient care concerns.

• Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.

• Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.

• Knowledge of electronic medical record.

• Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.

• Ability to follow standard precaution guidelines for infection control, including when providing patient care.

• Technical/Vocational School [Required]

• 3+ lpn experience [Required]

• Home health experience [Preferred]

• Licensed Practical Nurse (LPN) [Required]

• Basic Life Support - CPR Cert (BLS) [Required]

• Driver's License (DL) [Required]

• Certified Hospice and Palliative Nurse (CHPN) [Preferred]
