About the Role

As a Transitions Caseworker, you will play a key front-line role in supporting clients as they navigate pathways out of homelessness and into housing stability. Using a Housing First and Recovery-Oriented approach, you will work alongside individuals to identify barriers and build on personal strengths to promote long-term, sustainable recovery and independent living.

Working as part of our Housing Transitions Team, you’ll support clients through all stages of the housing process: from intake and move-in to long-term case planning and community integration. Some of your responsibilities would include:

  • Proactively engaging with clients to understand their current needs, barriers, and strengths
  • Supporting clients through personalized case management, goal-setting, and progress tracking
  • Assist with securing housing: scheduling viewings, lease signings, move-ins, and more
  • Help clients overcome practical barriers such as ID, bank accounts, rent payment, and furniture
  • Provide one-on-one support for life skills, recovery planning, and goal development
  • Collaborate with housing partners and community resources to support client success
  • Provide transportation to appointments, meetings, and housing-related activities using an agency vehicle
  • Ensure accurate documentation including case notes, stats, and housing-related forms
  • Conduct regular check-ins and maintain professional rapport with clients
  • Support transitional housing clients with move-ins, inspections, room cleaning, and guest management
  • Actively participate in team meetings, program evaluations, and community engagement
  • Follow all agency policies, regulatory standards (MSDS, OH&S, WHMIS), and Accreditation requirements
  • Be a positive ambassador for Alpha House and its values
