Job Summary
The Patient Care Coordinator I provides exceptional customer service and serves as a primary point of contact for patients, caregivers, nurses, physician offices, and internal departments. The PCC I performs routine patient coordination activities, including scheduling medication deliveries and collecting required clinical, insurance, and patient information while ensuring compliance with regulatory requirements and company policies. This role
supports continuity of care, patient satisfaction, and operational efficiency by following established departmental workflows and escalating complex or unresolved matters when appropriate.

Location: Laguna Hills, CA (This is an on-site position; remote work is not an option at this time.)
Schedule: Schedule to be determined based on business needs, with shifts scheduled between 5:00 AM and 7:00 PM PST. Occasional Saturday availability may be required.
Duties and Responsibilities
- Provide professional customer service through inbound and outbound communication with patients, caregivers, nurses, physician offices, and internal departments.
- Perform routine patient coordination activities in accordance with established departmental workflows and procedures.
- Review patient profiles, delivery schedules, medication storage instructions, and new patient admission information for accuracy and completeness before confirming deliveries.
- Schedule timely medication deliveries for AIC and home infusion patients, maintaining a scheduling window of at least 14 days whenever possible.
- Communicate and document supply coordination requests received from patients, caregivers, and nurses.
- Collect copayments through approved payment methods as needed.
- Conduct required patient assessments, collect key clinical and insurance information, and coordinate appropriate follow-up with pharmacists, nurses, or other departments.
- Ensure patients are connected with a pharmacist for medication consultation when required by law.
- Collaborate with Billing, Copay, Delivery, Pharmacy, and other departments to address routine issues that could delay scheduling or delivery.
- Identify and resolve routine patient coordination issues and escalate complex, urgent, unfamiliar, or unresolved matters to the appropriate senior team member or department leadership.
- Maintain complete, accurate, and timely documentation within pharmacy operating systems and electronic patient records.
- Routinely review personal documentation for accuracy and completeness and promptly correct identified errors.
- Review and prioritize assigned tasks and queues according to established departmental expectations.
- Meet departmental standards for productivity, quality, accuracy, customer service, and phone responsiveness.
- Support the timely and balanced scheduling of deliveries throughout the week and help minimize preventable last-minute scheduling.
- Maintain compliance with HIPAA, Board of Pharmacy requirements, applicable accreditation standards, company policies, and departmental procedures.
- Protect patient confidentiality in all verbal, written, and electronic communications.
- Participate in departmental training, quality improvement activities, workflow initiatives, and implementation of approved technology as assigned.
- Demonstrate accountability, professionalism, reliability, collaboration, and a patient-centered approach in all interactions.
- Foster a culture of accountability, integrity, collaboration, and service excellence.
- Additional duties and responsibilities in different areas of the department on an as-need basis or determined by management.
Required Qualifications
- Minimum of one year of customer service experience, preferably in a healthcare or pharmacy setting.
- Excellent verbal and written customer service and communication skills.
- Strong interpersonal, telephone, and relationship-building skills, with the ability to adapt communication to the caller’s needs.
- Fluency in English, including speaking, reading, and writing.
- Proficiency in Microsoft Word, Excel, Outlook, and other basic office tools.
- Typing speed of at least 30 words per minute.
- Strong organizational skills and attention to detail.
- Ability to manage multiple routine tasks and priorities in a fast-paced environment.
- Ability to follow established procedures, apply training, accept feedback, and seek guidance when handling complex or unfamiliar situations.
- Ability to work independently within established routine workflows while maintaining productivity, accuracy, and documentation expectations.
- Self-motivated and able to learn new processes quickly while maintaining a collaborative and team-oriented
approach.
Preferred Qualifications
- Specialty pharmacy, home infusion, healthcare coordination, or patient support experience.
- Pharmacy Technician, Medical Assistant, or related healthcare experience.
- Experience coordinating care for patients receiving IV, IVIG, biologic, injectable, or other specialty therapies.
- Experience using pharmacy software systems or electronic medical record platforms.
- Spanish-language proficiency.
