The Clinical Effectiveness & Outcomes Pharmacist role synthesizes pediatric evidence and performance data to inform, implement, and optimize clinical pathways, clinical protocols, and other clinical decision support (CDS) tools. It provides timely, evidence-based guidance in response to evolving factors such as drug shortages, new FDA approvals, and safety alerts, while ensuring accurate and safe medication build through collaboration with Information Systems (IS) analysts and clinical pharmacists. The position also evaluates medication use and pathway performance using reporting tools and dashboards, translating findings into CDS enhancements, pathway/protocol revisions, and provider communication. Additionally, it advances medication safety and regulatory compliance by implementing standardized practices and supporting medication oversight.

Doctorate of Pharmacy or Bachelor of Pharmacy (if obtained prior to 2000)

Board Certified Pharmacotherapy Specialist (BCPS) - Board of Pharmacy Specialities

1. Conducts comprehensive pediatric literature reviews to inform clinical pathways, clinical protocols, and clinical decision support (CDS) tools.

2. Provides real-time recommendations to inform clinical pathways, clinical protocols, and CDS in response to external factors such as drug shortages, new FDA approvals, and safety alerts.

3. Collaborates with IS analysts and clinical pharmacists to ensure version control, change tracking, and pre/post evaluation of medication-related pathway builds, ensuring accuracy in medications, dosing, and indications.

4. Conducts structured Medication Use Evaluations using EHR reporting tools to analyze prescribing patterns and identify opportunities for evidence-based care and clinical pathway/protocol development.

5. Leverages current medication evidence to inform and advance sepsis quality improvement strategies and clinical outcomes.

6. Analyzes pathway-specific metric dashboards to monitor adherence and drive targeted improvements in patient outcomes.

7. Performs pre- and post-implementation assessments of pathway and CDS impact using metrics such as antibiotic utilization, opioid use, alert overrides, and safety events.

8. Translates oversight committee decisions into Epic build requirements, pathway/protocol updates, provider education, and communication plans as appropriate.

9. Implements pediatric safety practices aligned with national best practice guidelines, and regulatory standards, including dose-range checks, formulation standardization, and high-alert medication oversight.

10. Contributes to hospital and system committees by offering subject matter expertise and delivering medication-related insights to support clinical decision-making.
