In this rewarding role, you will help individuals build independence, develop life skills, explore their communities, and create meaningful connections through personalized support.
What You'll Do:
As a Community Habilitation DSP, you will work one-on-one or in small groups to support individuals in achieving their personal goals and increasing their independence.
Activities may include:
Assisting with community errands and appointments
Encouraging healthy lifestyles through outdoor exercise and recreation
Teaching budgeting and financial management skills
Supporting participation in leisure and social activities
Assisting with volunteer and community engagement opportunities
Promoting self-confidence, decision-making, and relationship-building
Helping individuals learn and practice daily living skills

Qualifications:
High school diploma or equivalent required
Valid New York State driver's license required
Reliable personal vehicle available for use during service hours
Commitment to supporting and empowering individuals with disabilities
Positive, patient, and proactive approach to skill development and community inclusion
Strong communication and interpersonal skills
