Responsibilities
~1 min read- →Provide clinical oversight for clients enrolled in state-funded Medicaid waiver and personal care programs
- →Conduct client assessments and develop individualized care plans aligned with client needs and state program guidelines
- →Supervise, support, and provide clinical guidance to Home Health Aides and Personal Care Assistants
- →Ensure care delivery aligns with approved services, authorized care hours, state program requirements, and BAYADA standards
- →Coordinate with state case managers, Medicaid managed care organizations, families, caregivers, and other external stakeholders
- →Support client reauthorization processes and help ensure care hours remain aligned with approved program parameters
- →Review documentation for accuracy, completeness, timeliness, and compliance with applicable state and program requirements
- →Facilitate caregiver training, orientation, and competency documentation as required
- →Monitor quality of care and respond to incidents, complaints, changes in client condition, and service concerns
- →Conduct supervisory visits and follow up on client and caregiver needs
- →Serve as a clinical resource, client advocate, and partner to the Director and Client Services Manager
- →Participate in quality assurance, performance improvement, and audit-readiness activities
- →Build positive relationships with clients, families, caregivers, state partners, and community resources
- →Model The BAYADA Way and help build a positive, mission-driven, team-oriented culture
- Current Registered Nurse (RN) license in good standing in New Jersey
- Minimum of 2 years of RN experience
- Strong organization, documentation, communication, and problem-solving skills
- Ability to manage state program requirements, client needs, and competing priorities
- Ability to provide clinical leadership and support to caregivers in a community-based setting
- Comfort using computer systems for documentation, communication, and care coordination
- Commitment to serving clients and families with compassion, accountability, and respect
Nice to Have
~1 min read- Experience with Medicaid, Medicaid waiver programs, or other state-funded services
- Home care, Assistive Care, or community-based nursing experience
- Prior supervisory, leadership, or caregiver-management experience
- Knowledge of state-specific personal care program rules, authorizations, and documentation standards
- Experience coordinating with state case managers or managed care organizations
- Experience with caregiver training, competency validation, or quality oversight
- Passion for serving underserved clients and helping people remain safely at home
