About the Role

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client’s home or residential accommodation.

Responsibilities

→Support and education for clients with terminal diagnoses
→Education and support for families and carers
→Administration and preparation of s/c medications and CSCI for symptom management
→General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
→Accurate/ Timely documentation in all client records.
→Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
→Complete initial assessments/develop care plans to ensure best outcome for our clients.
→Monitor and update care plans to ensure appropriate care is provided
→Recognise and respond to clinical deterioration, with appropriate escalation.
→Promoting best practice and initiating quality improvement activities

What you bring

→Current AHPRA Nursing
→Minimum 2 years palliative care nursing experience, community experience desirable
→Post graduate qualifications (or working towards) in Palliative Care
→NDIS Clearance (or willingness to obtain one)
→Police Clearance with in 6 months (or willingness to obtain one)
→Use of own car – KM allowance provided
→Full drivers licence, reliable and comprehensively insured car
→CPR and Manual handling
→A desire to join a team committed to the care of others in a mission-based organisation
