Registered Nurse (RN) - Behavioral

• 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
• Great Healthcare benefits, and education reimbursement of $5,250 annually
• Additional compensation for visits over your caseload

• Develop a specific behavior health plan of care based upon comprehensive assessment, patient input and consultation with physician and incorporate patient’s language, cultural and psychosocial needs into the care plan
• Coordinates referrals for appropriate services/disciplines/DME/Supplies based upon skilled nursing assessment and physician’s orders
• Assures timely and appropriate reassessment or discharge as per agency requirements
• Actively participates in the REACT medication reconciliation program .
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver’s License and reliable transportation
