As a Wraparound Facilitator at Health Connect America, you'll help children, youth, and families develop individualized plans that build on their strengths, address their needs, and connect them to resources that promote long-term stability and success. Through collaboration, advocacy, care coordination, and community engagement, you'll play a critical role in helping families navigate challenges and achieve meaningful outcomes.

As a Wraparound Facilitator, you'll serve as the coordinator and leader of a collaborative support team focused on helping youth and families achieve their goals. Day to day, you can expect to:

  • Communicate with clients, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
  • Conduct outreach and engagement with newly referred families, including the initial phone contact and completion of the intake process.
  • Engage the family and assemble the child and family team, including the youth, caregivers/legal representatives, family members living in the home, service providers, involved agency representatives, and natural/informal supports.
  • Coordinate and facilitate child and family team meetings at least every 30 days, or more often as program requirements and family needs dictate. Document and maintain all information regarding the plan of care, revisions, and team meetings.
  • Ensure all phases and activities of the MI-informed wraparound process are conducted with high quality and fidelity, and remain accountable to the family team.
  • Draw on lived experience as a caregiver to provide caregiver-to-caregiver support and help families navigate child-serving systems.
  • Work with regional children’s mental health Community Engagement Specialists to identify and engage local community resources.
  • Provide System of Care (SOC) 101 and other required trainings to caregivers, new staff, and community partners.
  • Travel approximately 75% of the time, primarily local and statewide.

This is a highly collaborative, community-based role that focuses on helping children and families build strong support systems and achieve lasting success. You'll spend time meeting with families, coordinating services, facilitating team meetings, and advocating for the resources and supports needed to help youth remain successful in their homes, schools, and communities.

Success in this role requires strong organizational skills, effective communication, creativity, and a passion for empowering families. You'll frequently collaborate with schools, behavioral health providers, community organizations, and local support systems while helping families navigate complex service systems.

Depending on client needs, you may meet with clients and team members multiple times throughout the week and participate in an on-call rotation as required by the program.
