~1 min readDue to growth, we are adding a Pharmacist, Patient Consultation Services to our team.
We are hiring part-time pharmacists to support two primary coverage models:
- 2 x 10-hour shifts per week, Mondays and one other weekday (typically 8:30 AM to 7:00 PM CT)
- 5 x 4-hour shifts per week, Monday-Friday (typically 11:00 AM to 3:00 PM CT)
These schedules represent our primary staffing needs; however, shift assignments are not guaranteed to be limited to these hours. Additional coverage opportunities and alternative scheduling needs may arise throughout the year, particularly during high-volume periods.
The Pharmacist, Patient Consultation Services provides telephonic clinical consultations to specialty pharmacy patients receiving complex and high-cost medications. This position conducts patient assessments, performs medication reconciliation, identifies potential medication-related concerns, and provides education regarding medication use, disease state management, adherence, administration techniques, and side effect management. The position interacts regularly with patients, caregivers, pharmacists, pharmacy technicians, and other healthcare professionals involved in patient care.

Responsibilities
~1 min read- Conducts telephonic clinical consultations in accordance with Lumicera's Patient Management Program to support safe and effective use of specialty medications.
- Performs patient assessments and medication reconciliation to identify adherence barriers, potential drug interactions, medication-related concerns, and opportunities for clinical intervention.
- Provides education regarding disease state management, medication administration, storage requirements, treatment expectations, and side effect mitigation strategies.
- Answers clinical questions from patients and caregivers to improve understanding of prescribed therapies and support informed treatment decisions.
- Identifies patient-specific clinical concerns and escalates findings through established processes to promote continuity of care and patient safety.
- Documents patient consultations, assessments, interventions, and clinical recommendations in pharmacy systems in accordance with company policies, accreditation standards and applicable regulatory requirements.
- Maintains department productivity, quality, and service level standards through timely completion of assigned patient consultations.
- Supports quality improvement initiatives through identification, documentation, and reporting of medication errors and other patient safety concerns in accordance with company policies and procedures.
- Maintains current knowledge of specialty medications, disease state treatment guidelines, and emerging therapies relevant to supported patient populations.
- Adheres to applicable federal and state laws, pharmacy regulations, accreditation standards, and organizational policies governing specialty pharmacy practice.

Requirements
~1 min read- Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD required.
- Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or the ability to gain reciprocity within 4 months, may be required based on state of residence laws or client contracts. Refer to the Licensure and Credentialing Verification P&P for current detail.
- Experience counseling patients and caregivers regarding medication therapy, treatment expectations, and disease state management required.
- Experience documenting patient assessments, clinical interventions, and medication-related recommendations in accordance with pharmacy practice standards required.
- Experience evaluating medication regimens to identify potential drug interactions, adherence barriers, and other medication-related concerns required.
- Participate in, adhere to, and support compliance program objectives.
- The ability to consistently interact cooperatively and respectfully with other employees.
