Behavioral Health Social Worker

• →Conduct psychosocial assessments and behavioral health evaluations.
• →Develop and implement patient-centered treatment and discharge plans.
• →Provide crisis intervention, supportive counseling, and therapeutic interventions.
• →Coordinate care with psychiatrists, physicians, nurses, case managers, and community agencies.
• →Facilitate referrals to mental health, substance use disorder, and community support services.
• →Advocate for patients and families while promoting continuity of care and positive patient outcomes.
• →Maintain accurate documentation in accordance with hospital, regulatory, and compliance standards.

• Master’s degree in Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor, or related field.
• License required upon hire and maintained per state regulations; otherwise, state registration and/or supervision required.
• Current BLS certificate upon hire and maintain current.
• Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience.
• Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).
